Pursuant to the Health Insurance Portability and Accountability Act (HIPAA), Team Iron Will has developed a notice for clients , which provides a clear explanation of privacy rights and practices as it relates to private health information.
1. A Notice of Privacy Practices (NPP) that describes how Team Iron Will may use or disclose protected health information and an individual’s rights with respect to the privacy of that information, shall be prepared.
2. The notice must include an effective date, the title of the privacy official (or other privacy contact) and his/her email address, and any special notes that apply to Team Iron Will’s practices.
3. The NPP will also include a description of the instances in which advance authorization for use or disclosure may or may not be sought and a description of the steps that the client may exercise with respect to his/her own health information.
4. Each client will be notified that they may inspect and copy their health information, that they may request restrictions on the use or disclosure of their health information, that they may request amendments to his/her health information and that they may find out what disclosures have been made to outside persons.
5. The Executive Director shall promptly revise and distribute this notice whenever there are material changes to any privacy practices.
6. Team Iron Will shall make the notice available to each client no later than the date of first service delivery and make a good faith effort to obtain the individual’s written acknowledgment of receipt of the notice. If an acknowledgment cannot be obtained, the provider must document his or her efforts to obtain the acknowledgment and the reason why it was not obtained.
7. Client will sign the Notice of Privacy Practices upon applying for services through Team Iron Will. If his/her signature is not obtained, staff will document, in the client’s record , their good faith efforts to obtain such acknowledgment and the reason why the acknowledgment was not obtained.
8. Team Iron Will shall prominently post and make available the NPP on any website it maintains that provides information about its customer services or benefits.
9. Team Iron Will may email the notice to an individual if the individual agrees to receive an electronic notice.
